This Waiver and Release of Liability ("Agreement") is entered into between Foundation School of Martial Arts ("Organisation") and the undersigned participant ({name}), or if the participant is a minor, the participant's parent or legal guardian ("Guardian").

1. Acknowledgment of Risks

I acknowledge that participation in martial arts involves inherent risks and challenges, which may include, but are not limited to, physical injuries (such as sprains, fractures, bruises, or concussions), emotional and psychological injury, and, in rare cases, severe or life-threatening conditions. I voluntarily assume these risks and understand their potential consequences.

2. Informed Consent

As the Guardian, I consent to the child’s participation in martial arts training. I acknowledge that I have read and understood the nature and extent of the risks involved and accept these risks as inherent to martial arts training and activities.

3. Emergency Medical Treatment

In the event of an injury or medical emergency, I authorize Foundation School of Martial Arts to seek emergency medical treatment for the participant if I am not present or available. Reasonable efforts will be made to contact me or the designated emergency contact.

4. Adherence to Rules and Regulations

I agree that the participant will adhere to all rules and regulations provided by Foundation School of Martial Arts, whether delivered verbally or in writing. I understand that failure to comply with these rules may result in suspension or removal from the program.

5. Waiver of Liability

I release, waive, and discharge Foundation School of Martial Arts, its instructors, agents, and employees from any and all liability, claims, demands, actions, or causes of action whatsoever, directly or indirectly, arising from or related to participation in martial arts training or associated activities, including competitions or events. This waiver does not apply in cases of gross negligence or intentional misconduct.

6. Changes to Agreement

I acknowledge that Foundation School of Martial Arts reserves the right to make changes to this Agreement and will notify me of any changes in writing or electronically.

7. Legal Advice

I acknowledge that I have had the opportunity to seek independent legal advice regarding this Agreement. I also understand that I may ask questions or seek clarification regarding its terms before agreeing.

By completing and submitting this form, I confirm that I have read, understood, and agree to the terms and conditions outlined above.
